Graduate Requirements Worksheet:
This worksheet will help you to keep track of your progress and to remind you of Departmental and Graduate School requirements.
Progress Report Form:
Progress reports are due annually. Please submit completed progress reports before the announced deadlines. Progress report forms for other programs, such as BEES, MEES, and MOCB are acceptable if you are in one of those programs.
Qualifying Exam Form:
PHD students should use this form during Entomology qualifying exams. Following the exam, please return the completed form to Joanne Lewis. Also, after passing your exam, fill out the Application for Admission to Candidacy form found on the graduate school web site.
Exit Form:
To help us plan for graduation and to facilitate continued contact with you, please complete the exit form one month before your graduation.
